Spider Angiomas & Spider Naevi
Spider angiomas & spider naevi are common vascular lesions found on the face & chest of children & young adults. The most reliable way to treat spider angiomas is with a vascular laser. Most cases require one session. Treatments are pain free, especially with lasers that use cryogen sprays.

How do dermatologists diagnose spider angiomas?
In 99% of cases a clinical examination with a hand held microscope can diagnose these vascular lesions. Spider angiomas consist of a central ‘feeding’ spot (body of a spider), with branching ‘legs’. Pressure on these lesions causes them to disappear.
What is the best treatment for spider angiomas?
A vascular laser is the best way to treat spider angiomas, telangiectasias & broken capillaries. Laser targets the deep vessels, whilst sparing the upper layers of skin.
We use the following lasers to treat vascular lesions; DermaV, VBeam Prima & Perfecta, Sciton Joule, 755 Alex as well as the ExcelV in my Sydney office. My favorite lasers incorporate in-built cooling sprays, known as cryogen gas. They markedly reduce discomfort for patients during the treatment; especially good for treating spider angiomas in children.
What is the procedure like?
Laser light feels like a short snap. In some cases we double pulse, meaning 2 short snaps delivered over 1-2 seconds. All procedures are done in our accredited laser suites in Brisbane & Sydney. During the procedure you will have eye protection glasses on. Pain? About 2 out of 10.
What is recovery like?
After the procedure you will have some bruising that will last for 2-4 days. I prefer a bruising setting as this has a much higher chance of clearance, as compared to more conservative settings.
Will broken capillaries of spider angiomas & naevi recur?
Recurrence is rare. Partially treated spider angiomas may recur, & hence why we prefer bruising settings to get the job done within one session. For more mature patients, there may be a genetic susceptibility to spider angiomas, hence you may develop more over time; especially for females.

What are the possible side effects of laser treatment?
Side effects are super rare, but include-
- Prolonged swelling >72 hours
- Bruising
- Ulceration- approximately 5% of cases. May take 7 days to heal
- Scarring from the laser- approximately 1% of cases
- White spots or demarcation; especially on the chest. This is not a side effect, but occurs because of background sun damage.
When to treat spider angiomas in children?
This concept is important to understand; most children below the age of 12 don’t really care about spider angiomas or spider naevi (exceptions do occur, but most will not notice the spots or won’t be bothered by them). Hence we don’t recommend treatment until the child is concerned (most common scenario- parents care, kids don’t).
For adults, laser treatment is uneventful- goggle on, laser spot on target, 2 shots in 2 seconds, all over. For children, understandably, it can be eventful- especially with bright lights, nurses, & the hum of laser machines. No kid enjoys laser treatment, especially on the face. The long story short- if your child can understand the process we can treat, if they are not bothered or find the event traumatic- DEFER TREATMENT for a few years.
What causes spider angiomas & naevi?
In short, genetics. A predisposition to venous lakes is activated at a specific time in your life. It’s not controlled.
What happens if I don’t treat angiomas; do they go away?
Spider angiomas & naevi in children & young adulthood do not resolve, however angiomas in pregnancy may reduce, as estrogen levels fall.
What about spider angiomas in pregnancy?
In some cases, spider angiomas in pregnancy will resolve & involute– about 10% to 50% of lesions will do so postpartum. If you are pregnant, I suggest you wait 2-6 months postpartum before making the decision about spider angioma treatment; this especially applies to lesions on the chest (as de-demarcation may occur).
What is so special about angiomas & naevi on the chest?
It is not that lasers do not work, on the contrary, they work too well. Even with a 5 mm spot size, removing spiders on the chest will leave new skin (same color as the inner arm). This will give areas of demarcation which look worse than the angiomas themselves.
It’s not that it can not be done. We may have to perform 3-5 sessions of fractional lasers (& or vascular lasers) to blend the area in. Think carefully about removing angiomas on the chest, especially if you have background sun-damage.
Are they dangerous?
No. Spider angiomas & naevi are capillaries that are benign & do not require treatment, apart from their unsightly presence.
What else can look like a spider angioma?
A cherry angioma or an early atypical venous lake can look like spiders, however a dermatologist can tell the difference. Management for all three lesions are similar, namely vascular lasers.
How much is laser treatment?
Cost for spider angioma removal is $490 performed by a nurse, performed by a dermatologist, it is $690. A partial Medicare rebate may apply if performed by a specialist.
Most cases require only one treatment. Angiomas on the chest are tricky to treat, especially on the background of sun damage.
Medicare item number rebate only applies when the lesions are clinically significant & obvious at a distance of at least 3 meters. This distance should be objectively measured, but is dependent on the eyesight of a Medicare review committee.

What are some other ways to treat spider angiomas if you don’t have access to lasers?
Lasers are by far the most effective way of treating broken capillaries. Other treatments include-
- Sclerotherapy
- Diathermy or hyfrecation with pin-point needles
- Excision
- Intense pulse light or broad band light
Can creams make spider angiomas disappear?
No. Creams have no effect on spider angiomas. For super small blood vessels, topicals such as timolol, tranexamic acid & brimonidine can partially collapse vessels.
Davin’s viewpoint on treating spider angiomas & spider naevi
Vascular lesions such as spider veins, angiomas, broken capillaries & red birthmarks are best treated with special vascular lasers.
I prefer lasers with cryo cooling, the best is probably DermaV; it is more reliable than the Candela Prima laser. Cooling gives that extra level of protection to the epidermis, allowing for higher energy levels & greater patient comfort. In Sydney I use the ExcelV laser by Cutera. Most cases of spiders only require one session
BBL, broad band light, (not Brazilian Butt Lift) is another way to reliably treat spider angiomas, telangiectasias & broken capillaries – veins. We use the M22 or Sciton Joule for this job. The latter has templates, however one can use a hole punch & cut out a template on 4X folded paper. It works a treat to protect the surrounding skin.
Spider angiomas on the chest, on the background of sun damaged skin (common in Brisbane & Sydney), is very tricky to manage as lasers work too well. It’s like fixing a deep scratch on a 40 year old car; you’ll rectify the problem, but there will be color changes in the background. A solution is-
- Small spot (3-5 mm) size to target angiomas
- Blend in the surrounding skin (sun damage areas) with thulium lasers
- Blend in the redness with vascular lasers (big spot)
- 3-6 sessions over 6 months
